Home Care and Hospice Director

Responsible for administration, management, and long-range planning of the Home Care and Hospice Departments. Manages multiple disciplines which may include nursing services, physical therapy, occupational therapy, speech therapy, social work, respite care, and home helpers. Typically requires a bachelor's degree in nursing and five or more years of management experience in home care and hospice.: Represents the organization in the service area and directs a coordinated team effort for patient care and customer service. Directs, supervises, and governs the home care and hospice programs including in-patient care, home care, and bereavement follow-up. Administers and maintains quality assurance, environmental, and infection control policies in accordance with objectives, policies, and procedures. Prepares reports regarding the activities of the operation. Directs and supervises the work of staff in providing for the care and needs of patients. Develops, maintains, and implements policies and procedures. Hires, trains, and motivates a qualified staff. Confers and cooperates with management personnel in formulating administrative and operational policies and procedures. Reviews and analyzes expenditure, financial, and operations reports to determine requirements for assisted living operations. Approves requisitions for equipment, materials, and supplies.

Home Care Hospice Reimbursement Manager

Assures that all daily patient financial services functions are completed to ensure maximum cash flow, timely customer service with patient families and other staff, and compliance with all regulatory requirements of governmental agencies and third party payers. Manages and supervises the Patient Financial Services staff.: Manages the production of the initial monthly billing, the monthly revenue journals, and related reports for General Accounting. Assists Contract Services in the development of rates for specific facility, provider, payer, and physician contracts. Assists the Controller and the Director of Accounting with revenue rate projections for annual budget preparation. Investigates and evaluates changes in any government or third party payer regulation that may impact reimbursement. Assists in the development of standards and processes that promote effective and efficient operations in the Patient Financial Services area and related areas. Oversees the coordination of staff to maintain the timely flow of billing data, including insurance verifications, census reporting, production of billing, and collections. Acts as an educational resource for staff and management for reimbursement, billing compliance, and insurance issues. Manages resources, such as time, expenses, supplies, labor, etc., and develops staff resources through training, mentoring, coaching, etc. May participate with various organizations on a local or State level in relationship to hospice/health care reimbursement.

Call Center Nurse RN

Provides telephone triage to member of the health plan. Advises and instructs callers on proper health care after assessing the urgency of symptoms or complaints.: Answers telephone inquiries, assesses symptoms and provides assistance to members of the health plan in accordance with established policies, procedures and protocols. Provides clinical nursing evaluation and assessment and instructs callers to seek appropriate medical care. Notifies physician on call with pertinent medical information or if consultation is deemed necessary by nurse. Provides nursing referral to appropriate and available resources, including affiliated services, affiliated medical practice or community resource. Enters data into a computerized database and forwards facsimiles to appropriate health providers. Consults and facilitates after hours communication with physicians, nurses, hospital staff and outside agencies. Communicates with pharmacies and providers to refill medications or to clarify prescriptions. Typically requires graduation from an accredited School of Nursing as a Registered Nurse with two years of hospital or public health nursing experience. May provide health insurance eligibility, pharmacy benefits, provider information, and other information to callers.

Director Hospice

Typically requires graduation from an accredited School of Nursing, current licensure, and a minimum of two years of experience in an acute, home health or hospice setting.: Ensures individualized, appropriate hospice plan of care through effective utilization of the interdisciplinary team. Ensures appropriateness of patient admissions, discharges, and level of hospice care through consultation with interdisciplinary groups. Participates in the patient/family assessment process and care plan development. Coordinates physician instructions with nursing staff and internal and external hospital departments. Coordinates and manages the daily workload of assigned staff to meet the clinical needs of hospice patients and their families. Manages patient care expenses within established guidelines. Ensures compliance with State, Federal, and JCAHO regulations. Hires, develops, and motivates a qualified hospice care staff. Provides guidance and monitors the performance of the hospice care staff.

Resident Care Coordinator RN

Coordinates the work of all disciplines involved in completing Minimum Data Set (MDS) assessments and resident care plans according to regulatory requirements and guidelines. Provides assistance to the Nursing Director by providing in-service to staff, assessing competencies, and evaluating nursing staff. Plans, supervises, and evaluates the delivery of resident care and activities. Provides nursing care and ensures that nursing procedures are followed. Reviews resident care plans and conducts interdisciplinary care plan conferences. Typically requires completion of a nursing program at an accredited school of nursing as a Registered Nurse and a minimum of three years of nursing experience in a long-term care setting working with MDS. A valid nursing license is required.: Ensures that medical supplies and equipment are maintained on each unit. Oversees documentation to comply with Medicare or state reimbursement rules. Monitors medication passes and treatment schedules to ensure that they are properly administered. Interprets resident charts to accurately complete sections of the MDS. Performs quality improvement audits and assists in the implementation of quality assurance measures. Reviews resident charts for contradictions in information and researches any contradictions to determine what is valid. Reviews and investigates complaints and incident reports.

Hospice Volunteer Coordinator

Recruits, trains, schedules and coordinates volunteers for hospice programs. This position typically requires a college degree and related work experience.: Recruits, trains, schedules and coordinates volunteers for hospice programs. Maintains regular contact with patient/family support volunteers to discuss volunteers’ interactions with patients/families. Assesses volunteer’s working relationship with other team members. Keeps volunteers informed of any changes in the patient’s condition and plan of care while ensuring timely documentation by volunteers. Offers support to volunteers and provides additional information and resources to volunteers as needed. Consults with administrators and staff to determine organization needs for various volunteer services and plan for volunteer recruitment. Attends and participates in professional and community meetings to recruit interested parties for volunteer service. Creates volunteer job descriptions. Interviews, screens, and refers applicants to appropriate assignments. Orients and trains volunteers prior to assignment. Serves as liaison between administration, staff, and volunteers. Establishes positive working relationships with staff members and volunteers and reconciles personnel problems. Develops and provides written protocols to volunteers. Participates in fund-raising activities. Schedules and coordinates volunteer recognition and appreciation events. This position typically requires a college degree and related work experience. May maintain a database of volunteer workers and hours of work.

Lactation Consultant RN

Develops, implements and educates patients, families, and staff on proper breast feeding techniques and lactation.: Provides consultation and instruction in basic breast feeding routines and techniques to new mothers, both inpatient and outpatient. Provides support and problem solving to patients experiencing feeding difficulty with infants. This position typically requires a valid license as a Registered Nurse and is typically certified by the International Board of Certified Lactation Consultants (IBCLC). Provides instruction, written information and demonstration of the use of various types of breast pumps and other devices. Schedules outpatient visits and provides consultation for breast feeding problems following discharge. Documents nursing interventions, patient response, effectiveness and complications. Prepares equipment and educates patient and family in proper use. Observes patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents. Documents treatments and assessments in the patient’s medical record. May conduct breast-feeding education classes to the community.
