Health & Fitness Supervisor

Develops and promotes a wellness recreation program in private health club or gymnasium to improve health and physical fitness of individuals and improve fitness and to reduce the incidents of illness and disease. Typically reports to a manager and coordinates activities of recreation service personnel. To carry out the responsibilities at this level typically requires a degree in physical education.: Explains and demonstrates use of apparatus and equipment. Explains and demonstrates principles, techniques, and methods of regulating movement of body, hands, or feet to achieve proficiency in activity. Observes participants during workout to detect and correct mistakes. Explains and enforces safety rules and regulations. Teaches and demonstrates to individuals or groups beginning or advanced calisthenics and reducing or corrective exercises evaluating abilities of individual to determine suitable training program. Adapts recreation programs to meet needs of individual sponsor agency or institution. Introduces new program activities, equipment, and materials to staff. Trains personnel and evaluates performance. Interprets recreation services in meetings and organizational planning. May work in team with administrative or other professional personnel, such as those engaged in medicine, benefits, nursing, psychology, and therapy, to ensure that recreation is well balanced, coordinated, and integrated with special services.

Patient Placement Coordinator

Collects and analyzes data on patient admissions, transfers, and bed availability in specified clinical areas to improve patient care and maximize bed utilization. Typically requires licensure as a Registered Nurse with strong administrative skills and significant clinical nursing experience.: Coordinates bed assignments and utilization by reviewing admissions, transfer, and discharge statistics within a specialty area. Develops, coordinates, and implements daily plans for patient assignment to ensure appropriate levels of patient care, distribution of nursing unit workload, and physician preferences. Communicates with physicians and nurses regarding emergency and direct admissions based on bed availability. Evaluates the admission and transfer processes. Makes continual rounds on floors to make sure that discharges occur in the appropriate time period and to ensure that environmental services turn rooms around quickly and efficiently to improve bed utilization. Initiates elective precertification and coordinates urgent or emergent admission authorizations. Coordinates out of network admissions, lack of referral from primary care provider, no insurance, and other special admissions with attending physicians and health care staff. Acts as a negotiator in resolving and clarifying the true bed needs of the patients and may discuss case specifics with the admitting physicians. Typically knows the exact number of open beds for every floor and unit as well as specific needs for all admissions at any given time. Refers long-term diagnoses and other high risk diagnoses to case management to ensure that patients remain on proper clinical pathways.

Minimum Data Set Coordinator

Coordinates completion of Minimum Data Set (MDS) assessments and resident care plans according to regulatory requirements and guidelines.: Ensures that MDS Assessments and care plans are accurately completed according to regulatory requirements and established deadlines. Coordinates with other departments to collect and input the information required for the federally mandated MDS assessments. Monitors input to ensure accuracy, timeliness and compliance with reimbursement authorities and standards. Reviews clinical charts and condition of patients. Plans and coordinates resident's plan of care with other members of the interdisciplinary team and discusses resident condition with physicians, nursing staff and family members. Enters data in the computer database required for the MDS. Oversees documentation to comply with Medicare or state reimbursement rules. Leads care plan conferences and other interdisciplinary meetings. Interprets resident charts to accurately complete sections of the MDS. Reviews resident charts for contradictions in information and researches any contradictions to determine what is valid. Remains current with job related professional developments, regulations and standards. This job typically requires a degree in Nursing from an accredited college or university and a minimum of one year or experience working with MDS. May perform the duties of a Registered Nurse as assigned.

Resident Care Coordinator RN

Coordinates the work of all disciplines involved in completing Minimum Data Set (MDS) assessments and resident care plans according to regulatory requirements and guidelines. Provides assistance to the Nursing Director by providing in-service to staff, assessing competencies, and evaluating nursing staff. Plans, supervises, and evaluates the delivery of resident care and activities. Provides nursing care and ensures that nursing procedures are followed. Reviews resident care plans and conducts interdisciplinary care plan conferences. Typically requires completion of a nursing program at an accredited school of nursing as a Registered Nurse and a minimum of three years of nursing experience in a long-term care setting working with MDS. A valid nursing license is required.: Ensures that medical supplies and equipment are maintained on each unit. Oversees documentation to comply with Medicare or state reimbursement rules. Monitors medication passes and treatment schedules to ensure that they are properly administered. Interprets resident charts to accurately complete sections of the MDS. Performs quality improvement audits and assists in the implementation of quality assurance measures. Reviews resident charts for contradictions in information and researches any contradictions to determine what is valid. Reviews and investigates complaints and incident reports.

Transplant Nurse Coordinator

Provides and coordinates the clinical care of potential and actual transplant candidates and recipients. Manages data for transplant patients and coordinates the clinical management of post-transplant patients. Provides follow-up and care across the continuum from pre-transplant, during transplant and post transplant. Typically requires a bachelor of science degree in nursing, current licensure as a Registered Nurse, and at least three years of nursing experience. Demonstrated expertise in transplants is typically preferred.: Coordinates and provides advanced nursing care for patients before and after organ transplantation. Coordinates the evaluation process for pre-transplant (referral) patients to ensure effective use of hospital and clinical resources. Reviews and prepares patient histories, physical examination results, pertinent laboratory data, and patient psychosocial assessments. Facilitates patient progress from identification as a potential transplant candidate through the research or treatment processes to post-discharge care, serving as concerned advocate, educator, and point of liaison for the patient. Assesses post-transplant organ function and general health status and collaboratively determines need for additional assessment or intervention. Facilitates the scheduling of studies or procedures for diagnostic purposes utilizing fiscal responsibility for patient and hospital costs. Participates in the development and implementation of protocol and standards of care for the transplant patient in collaboration with the multidisciplinary team. Monitors the process and outcomes of routine care to determine the need for quality improvement. Coordinates transplantation processes when a donor is available including communication between physicians, the local organ procurement organization, patient/families, and the medical team involved. Completes necessary documentation according to the United Network of Organ Sharing (UNOS) guidelines including listing, status changes, and post-transplant follow-up information. Communicates with multidisciplinary team members, patients, and families regarding patient care/issues. Coordinates discharge plans for patients pre-transplant or post-transplant including appropriate referrals. Maintains patient transplant record with current data to facilitate communication. Educates patients along the continuum to assist understanding of processes and procedures surrounding pre-and post-transplant issues. Develops patient education materials or identifies external resources that are appropriate for patient education in all phases of transplantation care. Educates nurses and other multidisciplinary team members about patient care issues to promote positive outcomes for transplant patients. Documents care and treatment plans, patient response to treatment, and relevant correspondence. Interprets research and published data to nursing staff and colleagues concerning current issues in transplantation including those of a clinical, legal, ethical, or economic nature. Identifies areas for study and participates in and/or designs and conducts research. Serves as a consultant to nurses and other health care professionals regarding the care of the organ transplant patient. Serves as a community consultation resource through media, telephone, and transplant Internet links for information about transplantation and organ donation. May provide on-call coverage, including coordination of activities prior to transplant, and as medical triage for outpatients in collaboration with the physician on-call.

Laboratory Information Systems Coordinator

Coordinates and facilitates application system implementations and upgrades for information systems supporting laboratory functions. Develops, implements, and evaluates laboratory information systems. Provides training and education to application system users so that workflow can be simplified by new systems. Typically requires experience as a Medical Laboratory Technologist and experience in systems implementation and support.: Develops and presents training plans that will assist users in systems capabilities of the application. Participates in modeling system functionality and usability to support workflow. Determines required resources and measures quality of laboratory systems reports. Maintains laboratory systems documentation and develops related policies and procedures. Regularly updates system, operational, and department documentation. Recommends areas for process improvement. Prepares reports and analyses setting forth progress, adverse trends and appropriate recommendations and/or conclusions on subjects relating to laboratory system management.

Clinical Research Coordinator

Coordinates and participates in clinical research studies by performing a variety of complex activities involved in the collection, compilation, documentation, and analysis of clinical research data. Assists in the determination of guidelines for the collection of clinical data or administration of clinical studies. Coordinates and implements procedures for data collection from patient charts, medical records, interviews, questionnaires, diagnostic tests and other sources. Typically requires a college degree in health sciences and three years of clinical or research experience. : Codes, evaluates, and interprets collected data and prepares appropriate documentation. Obtains blood samples, cultures, tissues, and other specimens for laboratory analysis. Ensures compliance with protocol guidelines and requirements of regulatory agencies. Educates staff members regarding study design and time lines. Evaluates and interprets collected clinical data in conjunction with the principal researcher. Coordinates preparation of manuscripts for publication. Assists in the preparation of amendments to protocols or modifications to study design, as appropriate. Coordinates the development of forms, questionnaires, and the application of research techniques. Reviews journals, abstracts, and scientific literature to remain abreast of new developments. May schedule patients for study procedures.

Trauma Coordinator

Reviews, coordinates, and monitors activities related to a trauma center. This position typically requires a Bachelor's degree in a health related field.: Implements and coordinates trauma system plans and collects trauma center data. Monitors trauma facility compliance with contract requirements and notifies management of identified problems. Evaluates data and prepares periodic reports reflecting trauma systems operation and patient outcomes. Monitors pre-hospital care and triage patterns for consistency with established protocols. Serves as a liaison on trauma related matters with other organizations. Coordinates injury or illness prevention activities with external organizations. Coordinates prevention activities with trauma system hospitals. Conducts periodic reviews and makes recommendations for changes to the trauma system plan. Investigates and responds to questions or complaints regarding the trauma system. Participates in case review and the standardization of practice guidelines. Coordinates community trauma education and prevention programs. This position typically requires a Bachelor's degree in a health related field. May assist in the development of other specialty care programs as needs are identified such as pediatric care, burn care, etc.

Patient Education Coordinator

Develops, coordinates, and evaluates educational programs for the orientation and development of staff. Develops, plans, and implements a program of education for all facility personnel, including orientation of new personnel to facility and operational techniques. : Provides training in safe, effective delivery of patient care that assures compliance with standards outlined in the facility procedure manual and State and Federal regulations. Determines the effectiveness of orientation materials and procedures through direct observation and in collaboration with the Director of Nursing Services and Medical Director. Plans, coordinates, and conducts special in-service classes for facility personnel to implement new procedures, policies, equipment, and patient care techniques. Maintains library educational materials for facility personnel.

Preventive Health Coordinator

Develops, implements, and maintains programs to enhance the effective use of medical services, to promote preventive care, and to increase the frequency of positive health outcomes. Develops and implements provider education and meetings. Typically requires a bachelor's degree in health education and experience in delivering preventive health programs in a community, corporate, educational, or medical setting.: Prepares and analyzes reports. Develops health education materials by writing regularly scheduled articles in enrollee and provider newsletters, reviewing and revising brochures, facilitating translations and distributing materials. Provides member education on adult preventive health topics through participation at community health events and health forums. Writes, revises, and maintains policies and procedures for preventive health. May be dedicated to health education for specific chronic diseases. May develop, tabulate, and analyze enrollee interest and needs surveys. May write grant proposals to secure health promotion grants. May perform on-site reviews of practitioners and medical records relative to preventive health activities. May provide assistance in completing the preventive health portion of surveys conducted by external review organizations.
