Responsible for the development of disease management programs and for managing a department consisting of Disease Management Consultants and Disease Case Managers. Collaborates with other members of the disease management team to define the concept and develop the content of programs related to specified disease states. Develops clinical support material, interventions and measures to assess the effectiveness of each program. Develops and designs intervention strategies for practitioners and patients. Secures and reviews educational materials from various sources (e.g. professional organizations, government agencies, pharmaceutical manufacturers.) Ensures that pharmacotherapy treatment recommendations in the disease management program are consistent with formulary initiatives. Identifies and remains familiar with appropriate quality of life instruments for the specified disease states. Teach or instruct vocational or occupational subjects at the postsecondary level (but at less than the baccalaureate) to students who have graduated or left high school. Teaching may take place in public or private schools whose primary business is education or in a school associated with an organization whose primary business is other than education.
