Provides and coordinates individualized care and resources to elders, disabled, and others so that they may continue to live in their homes and in the community. Provides outreach, in-home, and community supervision to meet the comprehensive needs of elders, people with severe mental illness, addictions, and/or severe emotional disabilities who require care and resources in their homes. Assists in treatment planning, and coordinates individual assessments and care plans that may include counseling, family meetings, transportation, house cleaning, personal hygiene, providing meals, and other health and wellness related activities.: Manages and administers the home care program. Maintains confidential client files and budget and program information. Prepares and presents reports and information as required. Manages and distributes resources effectively, and ensures that all home care is provided in a caring and respectful way in keeping with all relevant policies and procedures and home care standards. Liaises with other community services and resources to ensure that other resources are involved, if required. Provides information and education to individuals and groups in the community, as required and requested. Provides intensive crisis support, family team building, and family education programs that serve to help preserve, strengthen, and/or restore the integrity of the family unit. Provides parenting education, skills development, supportive counseling to children and/or family, social work crisis intervention, self-esteem building activities, and substance abuse assessment, education, and referral. Provides tools for financial management, household management and preventative maintenance, preventive physical and mental health, educational/vocational assessment and planning, as well as resource generation, identification, and referral, developing linkages with other support systems. Checks on visiting aides and companions and ensures that services and deliveries are done on time and as promised. Maintains caseload and meets all deadlines for reports and documentation as required. Calls or visits clients to see how they are doing. Maintains care standards by hiring, training, motivating, and scheduling our staff of caregivers.

Directs organization's activities of strategic planning, evaluation, coordination, and implementation of various managed health care programs (HMO's, PPO's, long-term care). Organizes and develops short- and long-term planning programs to ascertain community and organization's requirements. Typically reports to Hospital Administrator and directs subordinate staff.: Administers fiscal operations such as budget planning and establishing rates for alternative delivery services. Directs activities concerned with contracts for alternate delivery systems. Reviews bids for conformity to contract requirements and determines acceptable bids. Negotiates contract with bidder. May recruit, hire, train staff, evaluate employee performance, and recommend or initiate promotions, transfers, and disciplinary action. Analyzes department budget to identify budget needs and or reductions, and may allocate operating budget's funds. Interprets company policy to employees and enforces safety regulations.

Intensive Care Executive

Manages the operations of all intensive care units.: Organizes and develops short- and long-term planning programs to ascertain requirements. Administers fiscal operations such as budget planning and establishing rates for services. Analyzes and evaluates the financial, operational and delivery aspects of intensive care programs. Ensures compliance with all regulations and license requirements. Develops policies and procedures for various activities to ensure that the objectives of the facility, physicians, care givers and patient are met. Recommends measures to improve methods, performance, and quality of service, and suggests changes in working conditions to increase efficiency. Analyzes department budget to identify budget needs and or reductions, and may allocate operating budget's funds. This position typically requires a registered nurse license and a minimum of five years of intensive care nursing experience. May recruit, hire, train staff, evaluate employee performance, and recommend or initiate promotions, transfers, and disciplinary action.

Patient Care Executive

Provides executive leadership for a broad range of patient care operations to include nursing and several other professional services departments such as rehabilitation, respiratory care, pharmacy, diagnostic imaging, and laboratory services.: This position typically reports to the CEO or COO of the organization. Plans, organizes, implements and evaluates outcomes of the patient care strategic plans. Develops strategic patient care initiatives and transforms initiatives into action plans. Performs regular data analysis of hospital census, quality data and operations cost reports to identify variances and their causes so that areas of focus may be identified and resources may be appropriately deployed. Works closely with the Medical Director to ensure that patient care services are provided in an efficient and effective manner. Provides oversight and direction in establishing and maintaining a system for developing, reviewing, approving and disseminating standards of clinical practice. Assists in reviewing and making recommendations on policy issues regarding clinical practice with appropriate discipline within the organization. Prepares and manages the departmental budget. Hires, develops and motivates a qualified patient care management staff. An advanced degree in a field of health or nursing is typically required for this position. The position typically requires strong clinical and quality improvement knowledge.

Child Care Center Director (Hospital run)

Oversees and directs the daily operations of a Child Care Center that is operated by a hospital system. Directs and supervises the work of staff in providing for the care and needs of children. Develops, maintains, and implements policies and procedures. Typically requires a bachelor's degree in a related field and five or more years of child care management experience.: Ensures that the Child Care Center meets all licensing regulations required for the care of children. Remains prepared for visits by the licensing consultant for licensing renewal. Manages day care billing and the maintenance of account records. Collects and reconciles program tuition, collects monies for food service, and deposits according to guidelines. Creates and updates a parent handbook and operations procedures manual. Maintains enrollment records and daily attendance records of children and staff. Prepares and implements developmentally appropriate lesson plans. Maintains regular communications with parents and develops programs responsive to parental needs and interests. Responsible for the safety of children in daily center activities and for field trips, if any. Home Care and Hospice Director

Responsible for administration, management, and long-range planning of the Home Care and Hospice Departments. Manages multiple disciplines which may include nursing services, physical therapy, occupational therapy, speech therapy, social work, respite care, and home helpers. Typically requires a bachelor's degree in nursing and five or more years of management experience in home care and hospice.: Represents the organization in the service area and directs a coordinated team effort for patient care and customer service. Directs, supervises, and governs the home care and hospice programs including in-patient care, home care, and bereavement follow-up. Administers and maintains quality assurance, environmental, and infection control policies in accordance with objectives, policies, and procedures. Product Development Health Care Director

Directs and oversees the assessment and development of potential products and business service opportunities. Plans and directs all aspects of the organization's product development policies, objectives, and initiatives. Typically requires a college degree in health services or marketing and three years of related product development experience.: Conducts financial feasibility studies and directs market research of potential products and business service opportunities. Reviews and analyzes proposals submitted to determine if benefits derived and possible applications justify expenditures. Managed Care Contracting Manager

Responsible for the coordination, evaluation and negotiation of managed care contracts under consideration. This position typically requires five or more years of experience in the negotiation of contracts in a managed care or hospital environment.: Responsible for the coordination, evaluation and negotiation of managed care contracts under consideration. Reviews and analyzes proposals submitted to determine if benefits derived and possible applications justify expenditures. Home Care Hospice Reimbursement Manager

Assures that all daily patient financial services functions are completed to ensure maximum cash flow, timely customer service with patient families and other staff, and compliance with all regulatory requirements of governmental agencies and third party payers. Manages and supervises the Patient Financial Services staff.: Manages the production of the initial monthly billing, the monthly revenue journals, and related reports for General Accounting. Assists Contract Services in the development of rates for specific facility, provider, payer, and physician contracts. Assists the Controller and the Director of Accounting with revenue rate projections for annual budget preparation. Investigates and evaluates changes in any government or third party payer regulation that may impact reimbursement. Assists in the development of standards and processes that promote effective and efficient operations in the Patient Financial Services area and related areas. Oversees the coordination of staff to maintain the timely flow of billing data, including insurance verifications, census reporting, production of billing, and collections. Acts as an educational resource for staff and management for reimbursement, billing compliance, and insurance issues. Manages resources, such as time, expenses, supplies, labor, etc., and develops staff resources through training, mentoring, coaching, etc. May participate with various organizations on a local or State level in relationship to hospice/health care reimbursement.
